LPN Continuous Care Specialist - Part-Time Position
Job Summary
Empath Health is seeking a skilled LPN to provide continuous care and support to patients and their families in times of crisis. As a LPN Continuous Care Specialist, you will work under the direction of a RN case manager to provide bed-side clinical observations, prescribed care, and emotional support to patients.
Key Responsibilities
- Provide continuous bed-side clinical observations and prescribed care to patients as directed in the plan of care.
- Accept and process physician orders to maximize patient comfort.
- Collaborate with the RN case manager to develop and implement patient care plans.
Requirements
Applicants must be a graduate of an approved nursing program and hold a current Florida LPN license. A minimum of one year experience in an acute care setting is required, with previous experience as a hospice LPN preferred.
