Job Summary:
Liberty Health is seeking an experienced Hospice Aide to join our team. As a Hospice Aide, you will provide direct patient care and support to individuals facing terminal illness.
Key Responsibilities:
- Provide high-quality patient care and support, including personal care and comfort measures.
- Document patient care and progress accurately and thoroughly.
- Collaborate with healthcare teams to ensure comprehensive care.
- Communicate effectively with patients, families, and healthcare professionals.
- Participate in ongoing education and training to enhance skills and knowledge.
Requirements:
- High school diploma or equivalent required.
- Current listing with the Division of Facility Services Registry.
- One year of patient care experience in an acute care or custodial setting.
- Documented proof of at least 75 certified education hours.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- CPR certification.
- Minimum of 12 hours of continuing education annually.
