Key Responsibilities:
  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans.
  • Manage patients through acute medical situations, chronic condition exacerbations, and short-term rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Collaborate with the interdisciplinary care team to provide seamless care coordination.
  • Conduct advanced illness and advanced care planning conversations to support patients and their families.
  • Provide patients and caregivers with education and counseling to promote healthy behaviors and self-management.
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board, with certification in one of the following areas: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, or Adult-Gerontology Acute Care Nurse Practitioner.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and performing physical assessments.
  • Access to reliable transportation.
Preferred Qualifications:
  • 1+ years of clinical experience in practice, preferably in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.

New York Residents Only: The salary range for this role is $109,513 to $147,146 per year. Pay is based on several factors, including local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
