We are seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team. As a member of our team, you will have the opportunity to work with a diverse range of patients and provide care that is tailored to their individual needs.

Key Responsibilities:
  • Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Ensure accurate and complete documentation of patient information, including ICD-10 conditions.
  • Communicate and collaborate with the interdisciplinary care team to provide seamless care.
  • Provide patients and caregivers with counseling and education to support their health and well-being.
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and a variety of dwelling conditions.
  • Access to reliable transportation.
Preferred Qualifications:
  • 1+ years of clinical experience in practice, preferably in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
