Position Overview

Sign-On Bonus: $20,000

Nuvance Health operates a comprehensive network of hospitals and outpatient facilities, providing a range of healthcare services across Connecticut and New York.

Role Summary:

The Occupational Therapist is responsible for delivering therapeutic services aimed at identifying, preventing, and rehabilitating physical dysfunction or discomfort, with a particular focus on enhancing upper extremity function and promoting independence in daily activities. This role involves ongoing interdisciplinary assessment and the development of tailored care plans in collaboration with patients and their support systems.

Key Responsibilities:

  • Collaborate with patients, their representatives, and healthcare providers to create, implement, and assess individualized care plans.
  • Deliver therapeutic services as prescribed by physicians, ensuring effective communication with all involved healthcare professionals.
  • Provide direct occupational therapy interventions in alignment with established care plans.
  • Maintain comprehensive records of client visits, including evaluations, care plans, and responses to therapy, and facilitate discharge planning as necessary.
  • Engage in interdisciplinary case management meetings to ensure cohesive integration of occupational therapy within the overall care strategy.
  • Educate and counsel patients and their families regarding occupational therapy processes and expected outcomes.
  • Identify additional home health service needs for patients and facilitate appropriate referrals.
  • Contribute to quality assurance initiatives and participate in agency-sponsored training programs.

Qualifications:

  • Valid Driver's License and access to a reliable vehicle.
  • Bachelor's Degree in Occupational Therapy.
  • Current Occupational Therapy License in Connecticut.
  • Minimum of one year of relevant experience.

Work Environment:

This is a full-time position with standard working hours of 40 hours per week, Monday through Friday, from 8:30 AM to 5:00 PM.

Compensation:

Hourly Rate: $65.01
