Role Summary:

The Occupational Therapist is responsible for delivering therapeutic services aimed at identifying, preventing, and rehabilitating physical dysfunction or discomfort, particularly focusing on enhancing upper extremity functionality and promoting independence in daily activities. This role involves collaborating with patients, their representatives, and caregivers to develop and implement personalized care plans, while maintaining effective communication with the healthcare team.

Key Responsibilities:

  • Formulate, execute, and assess individualized care plans in collaboration with patients, their representatives, and healthcare providers.
  • Deliver therapeutic services as prescribed by the physician, ensuring adherence to the established care plan.
  • Maintain comprehensive documentation of client interactions, including evaluations, care plans, and therapy outcomes.
  • Engage in interdisciplinary case management discussions to align occupational therapy services with the overall care strategy.
  • Provide education and support to patients and their families regarding the occupational therapy process and treatment plans.
  • Identify additional healthcare needs for patients and facilitate referrals to appropriate services as necessary.
  • Contribute to quality assurance initiatives and participate in ongoing professional development training.

Qualifications:

  • Valid Driver's License and reliable transportation.
  • Bachelor's Degree in Occupational Therapy.
  • Current Occupational Therapy License in the state of practice.
  • Minimum of one year of relevant experience in the field.

Work Environment:
