Primary Care Physician Assistant or Nurse Practitioner

About the Role
The Wonderful Company is seeking a highly skilled Physician Assistant or Nurse Practitioner to join our team as a Primary Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality primary and preventive care to our patients, emphasizing continuity and developing lasting caring relationships.
Key Responsibilities
- Provide comprehensive outpatient primary care to adults and children, including diagnosis, treatment, and management of complex chronic medical conditions.
- Collaborate closely with a team of healthcare professionals, including a physician and health coaches, to ensure seamless patient care.
- Coordinate and grow a network of trusted outside medical consultants, hospitalists, emergency room providers, and support services.
- Lead or support onsite and community-based efforts to drive patient engagement, access, population-based patient management, referral tracking, and clinical quality performance evaluation.
- Confer with the supervising physician on cases agreed upon in the delegation contract.
- Travel to various clinic locations, including Lost Hills, Delano, Student Wellness Center, and Mobile clinic.
Requirements
- Current, valid California Physician Assistant (PA) license or California Nurse Practitioner (NP) license.
- Valid DEA registration.
- Proficient with technology, including MS office, tablets, smartphones, and EMR systems.
- Written and verbal fluency in Spanish strongly preferred, Medical Spanish a must.
