Job Summary

We are seeking a skilled Social Work professional to join our team at Mass General Brigham in Brighton, Massachusetts. As a Social Worker, you will play a crucial role in assessing patients and families from a social work and case management perspective, completing necessary clinical documentation, providing education on insurance coverage and community resources, making referrals to appropriate agencies, and participating in interdisciplinary team processes.

This is a dynamic position that requires strong communication and interpersonal skills, the ability to work independently, and a high level of service delivery. The estimated salary for this role is $65,000 - $85,000 per year, depending on experience.

About the Role

As a Social Worker at Mass General Brigham, your key responsibilities will include:

  • Completing clinical social work assessments and MDS entries within established timeframes
  • Developing case management/social work plans collaboratively with the interdisciplinary team, patient, and family
  • Submitting clinical reviews to third-party payors/insurers in a timely manner
  • Culturally competent care to identify patient cultural needs and incorporate them into discharge plans
  • Patient education and support to ensure a smooth transition from acute settings
Requirements

To be successful in this role, you will need:

  • A current Massachusetts license as LCSW/LICSW
  • Knowledge of community services and healthcare systems
  • Computer proficiency with Microsoft Office applications
  • Highly developed communications and interpersonal skills
  • Ability to work independently and contribute to a team
  • Attention to detail and ability to prioritize work
