Social Worker and Case Manager Professional

We are seeking a highly skilled Social Worker and Case Manager to join our team at Mass General Brigham. This is a dynamic opportunity to work in a collaborative environment where you will have the chance to make a meaningful impact on patients' lives.
About the RoleThis full-time position involves assessing patients and families from a social work and case management perspective, completing necessary clinical documentation, providing education to patients/families regarding insurance coverage and community resources, making referrals to appropriate community agencies/resources, and participating in interdisciplinary team processes/collaborating with the interdisciplinary team to formulate treatment plans to maximize emotional, social, and physical well-being.
Responsibilities- Complete clinical social work assessments and MDS entries within established timeframes, ensuring all assessment elements conform to Clinical Social Work Scope of Practice guidelines.
- Develop a case management/social work plan collaboratively with the interdisciplinary team, patient, and family to maximize healthcare responses, quality, and cost-effective outcomes, incorporating patient and family goals, expectations, and preferences into the development of the interdisciplinary plan of care.
- Submit clinical reviews to third-party payors/insurers in a timely manner.
- Provide culturally competent care by identifying patient cultural needs and incorporating them into the discharge plan.
- Offer patient education and support to ensure a smooth transition from an acute setting through the SRN stay and transition to discharge using established standards.

- CURRENT MASSACHUSETTS LICENSE AS LCSW/LICSW
- Knowledge of community services and healthcare systems required.
- Computer proficiency required; Microsoft Office applications preferred, with the ability to learn new software.
- Highly developed communications and interpersonal skills, working with diverse populations.
- Ability to work independently, be self-directed, and contribute as a member of a team.
- Anticipates challenges and develops strategies for addressing them.
- High level of service delivery, demonstrated initiative, with the ability to prioritize work, meet deadlines, and adapt to changing situations.
