Job Summary:

The LPN Nurse Consultant is responsible for delivering Wellness Coordination services as defined by the 5th amendment to the Indiana Code IN.0378.R02.05. This includes conducting training sessions in CPR/First Aid and Wellness plans, as well as providing Wellness Coordination services to monitor health status, high risk plans, and medication administration for assigned program participants.

  • Must possess a current Indiana nursing license; LPN
  • Two or more years experience working with individuals with disabilities preferred

We offer a competitive salary of $70,000-$90,000 per year, depending on experience, plus benefits such as dependent transportation, valid Indiana driver's license, and proof of personal automobile insurance. If you are a skilled communicator with excellent verbal and written skills, and can make presentations and write concise reports, we would love to hear from you.
