Job Title: Home Health Nurse - Pediatric and Adult Patients

About the Role:
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals in providing high-quality patient care. As a home health nurse, you will work with pediatric and adult patients in their own homes, assessing, planning, implementing, and evaluating patient care to ensure optimal outcomes.

Responsibilities:
- Utilize the nursing process to assess patient needs and develop individualized care plans.
- Provide skilled nursing care and services in accordance with prescribed orders from physicians and other healthcare professionals.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of service.

Requirements:
- Current licensure as an LPN/LVN in the state where you will practice.
- TB or Chest X-Ray within the past year.
- BLS certification is required.
- One year prior professional nursing experience is preferred but not necessary.
