Job Summary

Family Health Centers at NYU Langone is seeking a skilled Weekday Nurse Coordinator to join our team. In this role, you will coordinate patient care in the Department of Dental Medicine, ensuring seamless interactions between patients, providers, and insurance companies.

Key Responsibilities

  • Coordinate all levels of patient care, including pre-authorization and treatment planning
  • Act as a facilitator on behalf of the Department to resolve patient care issues
  • Ensure appropriate assignment of patients to providers and monitor care to ensure positive clinical outcomes
  • Prepare statistical reports on services provided and develop patient education models and programs

Requirements

  • 3+ years of experience in clinical care coordination and/or treatment coordination
  • Patient interaction and care coordination experience
  • FQHC or Hospital experience preferred
