Hospice Registered Nurse

A prominent healthcare organization is seeking to enhance its Hospice team by adding a qualified Registered Nurse (RN) in a community-focused role. The selected Hospice Nurse (RN) will engage with patients in their residences, delivering essential palliative care services tailored to individual needs.
Position Overview:
- Specialization: Hospice Care
- Target Demographic: Adult and Elderly Patients
- Work Schedule: Weekdays
- Working Hours: Standard business hours
The Hospice Nurse (RN) will be tasked with:
- Conducting comprehensive physical and emotional assessments
- Documenting patient visits electronically
- Creating and executing individualized nursing care plans
- Advising on care modifications based on patient evaluations
- Coordinating with physicians to obtain necessary written orders for care adjustments
- Overseeing, mentoring, and assessing Home Health Aides and support staff involved with hospice patients
- Carrying out additional responsibilities as required
- Prior experience in hospice care or a minimum of one year in a clinical environment
- Graduation from an accredited Nursing program
- Valid New York State Registered Nurse (RN) license
- Proficiency in Cantonese or Mandarin is preferred
- Background in Hospice and/or Home Health Care
