Care Coordinator for Acute Medical Patients

About the Role:
This contract position offers an estimated salary of $120,000 per year, based on national averages for Registered Nurses in the United States. You will be working in a dynamic, fast-paced environment, providing high-quality patient care and collaborating with a multidisciplinary team of healthcare professionals.
Responsibilities and Qualifications:
- Perform initial and ongoing assessments of patients, monitoring vital signs, cardiac rhythms, and respiratory status.
- Develop individualized care plans based on assessment data, focusing on both medical and telemetry care needs.
- Administer medications, interventions, and therapies in alignment with care plans.
- Continuously monitor patients for changes in status, particularly with regard to cardiac rhythms and pulmonary function.
- Floating to like units as necessary, including ICU or ED, to provide support and care for Med-Surg level patients.
Qualifications:
- Licensure: Must have a current State Registered Nurse License.
- Experience: Minimum of 1 year (preferably 2 years) of Med-Surg and Telemetry experience.
- Certifications: BLS (Basic Life Support) certification required.
