About the Job

Boro Park Center for Rehabilitation and Healthcare is seeking a skilled Admissions Registered Nurse to join our team. As an Admissions Registered Nurse, you will play a vital role in ensuring the smooth transition of new residents into our facility.

Key Responsibilities

  • Initiate admission assessments on new residents and hospital returns, and develop initial care plans.
  • Assist with the development and direction of the Nursing Department as directed by the DON.
  • Review Priorities of Care (PRI's) and coordinate all components of the admission process with the interdisciplinary team.
  • Audit records to ensure compliance with facility standards.
  • Participate in discharge planning as necessary.
  • Review and evaluate residents' care plans when conditions warrant a change.
  • Perform required charting according to policy and procedure.
  • Complete Minimum Data Set (MDS), care plans, and other documents as assigned.
  • Maintain the confidentiality of all resident information as per HIPAA and Corporate guidelines.
  • May be assigned to supervise or direct resident care as necessary.

Requirements and Qualifications

  • Must have an active and unrestricted NYS Registered Nurse License.
  • Prior experience as an admissions nurse in Long-Term Care (LTC) settings is preferred.
  • Excellent clinical assessment skills and ability to make good independent decisions are essential.
  • Strong leadership and supervisory skills are required.
  • Computer proficiency in MS Office (Excel, Word, Outlook) is necessary.
  • Long-Term Care experience is a plus.
  • Excellent communication and interpersonal skills are required.
