REPORTS TO: Director of Prevention and Compeer (MHASCK)
POSITION SUMMARY: The Community Care Navigator will engage with residents in Nursing Homes for Mental Health throughout Kansas who are eager to transition back into community living. By employing a person-centered methodology, the Navigator will oversee care coordination, collaborating with residents, guardians, NFMH Liaisons, CMHCs, MCOs, and KDADS to deliver comprehensive support services for clients interested in leaving an NFMH. Responsibilities also include gathering participant data, managing cases, planning discharges, and advocating for clients. This role necessitates regular travel and participation in relevant meetings as required.
KEY RESPONSIBILITIES:
- Deliver services to achieve program objectives, managing a caseload of around 30 clients concurrently.
- Ensure timely completion and accuracy of various reports, including mileage and timecard submissions.
- Maintain precise and prompt documentation of service delivery, crafting individualized case notes that reflect suitable interventions and progress.
- Facilitate the creation of a Person-Centered Care Plan with clients, families, and CMHC NFMH Liaisons, utilizing the Preadmission Screening and Resident Review (PASRR).
- Conduct periodic evaluations of Care and WRAP plans to track client progress.
- Develop crisis management strategies with clients using the WRAP framework.
- Monitor client status and provide appropriate levels of support, notifying relevant parties of any changes.
- Coordinate additional services necessary to meet identified goals aligned with client needs and functioning levels.
- Collaborate on discharge planning with clients, MCOs, CMHCs, NFMHs, KDADS, and caregivers.
- Attend all required departmental and agency meetings and training sessions.
ADDITIONAL RESPONSIBILITIES:
- Maintain a satisfactory attendance record, providing appropriate notifications for absences and ensuring coverage of responsibilities.
- Demonstrate a solid understanding of technical knowledge relevant to the position.
- Work collaboratively within a team, keeping colleagues informed and treating others with respect.
- Exhibit the ability to assess family needs effectively.
- Engage in professional verbal and written communication with peers, clients, supervisors, and other staff, employing diplomacy in challenging situations.
- Show adaptability to new tasks and changes in procedures, taking initiative to identify and address necessary actions.
- Perform duties with diligence and a high degree of accuracy.
- Complete assigned tasks and meet deadlines independently.
- Achieve goals and objectives as established during performance evaluations.
QUALIFICATIONS: Candidates must hold a Bachelor's degree in a behavioral health discipline and possess a minimum of three years of relevant experience. Preference will be given to those with experience working with individuals facing dual diagnoses of substance use and mental health issues, or those familiar with NFMHs and/or CMHCs. A valid Kansas driver's license and compliance with background checks are required. Frequent travel may be necessary.
