Flexible, Monday to Friday, 8:15 AM - 5:00 PM

YOUR IMPACT:
As a Physical Therapist (PT), you will play a crucial role in providing care and compassion to our patients.

You'll collaborate with an interdisciplinary team dedicated to delivering high-quality, compassionate care and achieving positive outcomes for our patient population.

Your responsibilities will include delivering home-based therapy as prescribed by the attending physician.

Engaging with patients' families while supporting your patients, you will have the rewarding opportunity to be part of their recovery journey.

YOUR ROLE:
You will deliver comprehensive skilled services as directed by the attending physician. You will educate patients and their families while ensuring patient safety. You are responsible for maintaining accurate documentation and adhering to all legal regulations.

KEY RESPONSIBILITIES:

Care Planning:
Provide skilled physical therapy as prescribed by the attending physician.

Monitor, document, and communicate patient reactions, changes, and progress to the attending physician and relevant agency staff.

Conduct OASIS assessments to create personalized care plans and adjust them as necessary.

Patient Outcomes:
Administer treatments to alleviate pain, restore function, and maintain optimal performance through physical means such as exercise, massage, heat, and water therapy.

Risk Management:
Reduce the likelihood of re-hospitalizations by prioritizing visits for high-risk patients and educating them on preventive measures and decision-making.

Family Education and Advocacy:
Guide patients and their families on managing diseases or injuries throughout all phases of physical therapy.

Instruct patients and family members on the proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other assistive devices.


Supervision:
Oversee and direct therapy services provided by Physical Therapist Assistants (PTAs) and, when appropriate, nursing assistants.

Communication and Collaboration:
Engage with PTAs and nursing aides regarding supervisory visit outcomes.

Compliance:
Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.

Regulatory Adherence:
Understand and comply with agency policies, procedures, rules, and regulations, and communicate any changes in schedule or availability to schedulers or supervisors.

Operational Duties:
Participate in in-service training and mandatory agency meetings.

SKILLS REQUIRED:
Compassionate communication with a positive demeanor
Patience when interacting with patients, families, physicians, and colleagues
Attention to detail and the ability to follow directions meticulously.

QUALIFICATIONS:
Licensed Physical Therapist in the state of employment. Valid driver's license and auto insurance required.

Ability to meet all physical demands.
