Position Summary
This is a part-time role, requiring 48 hours per pay period. The Licensed Practical Nurse (LPN) will be responsible for assessing injuries and implementing or recommending treatment in accordance with licensure and delegation protocols.

Key Responsibilities
- Serve as a liaison between participant families, healthcare providers, pharmacies, and relevant agencies.
- Collaborate closely with the Medical Director and Manager of Medical Services to ensure effective case coordination.
- Assist the treatment team in monitoring the execution of treatment plans to guarantee adherence to established protocols.
- Organize and gather information for treatment plan meetings.
- Support Direct Clinical Management with timely report writing and distribution.
- Document all clinical and case management activities in compliance with program, company, participant, legal, and regulatory standards.
- Manage the collection and storage of participant and program records in accordance with all applicable requirements.
- Participate in on-call crisis rotation as needed.
- Facilitate family visitation processes when applicable.
- Work collaboratively within a team framework, involving guardians and caregivers in care decisions whenever possible.
- Deliver services that honor the cultural backgrounds and identities of participants and their families.
- Engage proactively in public relations with families, employees, and community members, maintaining professionalism at all times.
- Coordinate necessary documentation for audits and conduct program tours for visitors and involved agencies.
- Represent the organization at conferences, job fairs, and community events as required.

Confidentiality and Compliance
- Uphold the highest standards of professional conduct regarding confidential information.
- Share information only when the recipient's right to access is clearly established and in the best interest of the participant.
- Adhere to the company's Health Insurance Portability Accountability Act (HIPAA) privacy practices and Business Ethics and Compliance programs.

Qualifications
- This is a part-time position, requiring 42 hours per pay period.
- Licensed Vocational Nurse (LVN) preferred; other licensed medical disciplines will be considered.
- Must maintain an unrestricted license to practice as a Licensed Vocational Nurse (LVN) or equivalent medical discipline in the assigned state(s).
- Current CPR certification is required.
- Excellent organizational, time management, and computer skills are essential.
- Strong interpersonal, verbal, and written communication skills are necessary.
- Ability to work independently as well as collaboratively within a team.
