We are seeking a highly skilled and compassionate Social Worker II to join our team at Houston Methodist Cypress Hospital. As a key member of our healthcare team, you will provide comprehensive psychosocial assessments, diagnosis, and treatment to patients and their families, ensuring seamless transitions and optimal outcomes.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patient needs and develop effective treatment plans
- Collaborate with healthcare teams to ensure comprehensive care and address social and emotional aspects of patient illness
- Develop and implement discharge plans to ensure safe and efficient transitions
- Provide education and support to patients and families to promote self-care and independence
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Master's degree in Social Work from an accredited university
- State licensure as a Licensed Medical Social Worker (LMSW)
- Certification as a Certified Social Worker in Healthcare (C-SWHC)
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced healthcare environment
