Job Summary

We are seeking a highly skilled Case Manager to join our Crisis Response Team at AllHealth Network. As a Case Manager, you will play a critical role in providing compassionate and integrated behavioral health services to individuals and families in our community.

Key Responsibilities
  • Assist clients with complex issues and link them with mental health resources
  • Work in partnership with a medical clinician to divert mental health, substance use, and medical emergencies from law enforcement response when legal intervention is unnecessary
  • Develop and implement care plans to meet the unique needs of each client
  • Provide and document clinical care interventions to meet regulatory standards and/or contractual requirements
  • Coordinate care with the Co-Responder Program's Case Managers, Co-Responders, and other AllHealth programs as appropriate and available based on client needs
Requirements
  • Bachelor's degree in a health/behavioral health related area of focus such as social work, psychology, sociology
  • Minimum of 2 years of related experience with diverse populations in a physical and/or behavioral health setting
  • Experience in case management, care coordination, and/or community health required
Preferred Qualifications
  • Participation and successful completion of Co-Responder/Case Manager Safety regimen
  • Participation and successful completion of the 40-hour CIT Officer Training
