Position Overview
We are seeking a full-time RN Care Manager to become an integral part of our team. This role demands a comprehensive understanding of the clinical, financial, administrative, and psychosocial aspects of healthcare delivery. The RN Care Manager will be responsible for conducting assessments, developing care plans, implementing interventions, and ensuring follow-up for patients and their families throughout the care continuum.
Work Schedule: Full-time, Monday to Friday (in-person)
Holidays: 9 recognized holidays annually
Compensation: The salary range for this position is $32.81/hr - $56.25/hr, with placement within this range determined by qualifications, professional experience, and educational background.
Key Responsibilities
- Ensure optimal outcomes are achieved in accordance with established standards of practice.
- Collaborate effectively with patients, families, caregivers, and the healthcare team to communicate, problem-solve, and share accountability for achieving the best outcomes.
- Coordinate care plans to meet desired outcomes while integrating the efforts of the healthcare team, including the patient's Primary Care/Medical Home.
- Facilitate the arrangement of services among community organizations, healthcare providers, patients, families, caregivers, and other stakeholders involved in the care plan.
- Advocate for patients by supporting or recommending services that protect their health, safety, and rights.
Qualifications
Required
- Bachelor's degree in Nursing
- Active RN licensure in Washington
- A minimum of 3 years of nursing experience
- Case Management Certification (CMC, ACM, or ANCC); if not currently certified, certification must be obtained within 12 months of hire/eligibility
- COVID-19 Vaccination
- Master's degree in Nursing
- 3-5 years of experience in Care Coordination/Case Management
- 5+ years of clinical experience in Pediatrics, Burn care, and/or Orthopaedics
- Experience in utilization review/management with knowledge of InterQual criteria
