About This Opportunity

We are seeking a highly skilled Home Health Physical Therapy Assistant to join our team at Andwell Health Partners. As a member of our team, you will have the opportunity to make a meaningful impact in the lives of our patients and their families.

Key Responsibilities
  • Develop and implement comprehensive physical therapy plans for patients in accordance with their individual needs and goals.
  • Collaborate with our healthcare team to ensure seamless care coordination and effective communication.
  • Provide expert physical therapy services to patients in their homes, focusing on relieving pain, restoring function, and promoting overall well-being.
  • Teach patients and their caregivers various techniques to manage daily activities, transfers, and positioning, empowering them to take an active role in their recovery.
  • Work closely with internal and external resources to identify and address patient outcomes, ensuring optimal results.
Requirements
  • Graduate of an accredited Associate Degree Program for Physical Therapy Assistants.
  • A minimum of one year of recent clinical experience.
  • Current license in the State of Maine as a Physical Therapy Assistant.
  • Reliable automobile with a current Maine driver's license.
