Job Summary

We are seeking a highly organized and detail-oriented Healthcare Support Specialist to join our team at Cape Abilities. As a key member of our staff, you will provide administrative support to our program case management team.

Key Responsibilities:
  • Provide home visit relief to nurses and case managers as needed.
  • Manage a small case load and perform case manager duties for that caseload.
Additional Duties:
  • Audit charts for completion.
  • Complete and update identification sheets and photos.
  • Follow up via telephone to request missing caregiver logs/financial transaction records (FTR) and other essential case file information.
  • Track six month summaries and physician's orders.
  • Track fire drills, home inspections, furnace inspections, and safety plans.
  • Track caregiver trainings.
  • File paperwork.
  • Track Individual Service Plans (ISP) and participant census via caregiver logs.
  • Track selected individuals' financial information (RFMS).
  • Serve as key contact for Adult Foster Care determination information.
  • Complete paperwork for new program participants/caregivers.
  • Ensure adequate supply of commonly used forms.
  • Manage incoming and outgoing mail.
  • Maintain daily staff schedule. Update the on-call list.
  • Order supplies.
  • Maintain updated relief list.
  • Calculate caregiver respite balances and submit check requests to management for approval.
  • Provide information and referrals.
  • Serve as mandated reporter for suspected abuse and/or neglect of individuals to the Disabled Persons Protection Commission (DPPC).
