## Job Summary

We are seeking a compassionate and skilled Hospice Aide to join our St. Croix Hospice team. As a valued member of our interdisciplinary care team, you will provide quality patient care, working under the supervision of a Registered Nurse. Your primary focus will be on delivering timely and accurate documentation of care, ensuring the highest level of patient satisfaction and comfort.

## Job Responsibilities

* Provide patient care, including bathing, nail/skin care, dressing/undressing, dental, and other personal hygiene, while maintaining a sanitary and orderly environment.
* Offer positive, respectful, and confidential patient/family communication.
* Take and record vital signs, and assist with self-administered medications.
* Plan and prepare nutritious meals, including shopping and feeding the patient as needed.
* Assess and report patient's condition and changes to the RN Case Manager.
* Assist in ambulation, exercise, transfers, positioning, and support a complete range of motion and other therapy activities.

## Qualifications

* At least 18 years of age.
* Active Certified Nursing Assistant (CNA) designation current with state-specific requirements.
* Licensed and insured driver with automobile in good working order.
