Job Title: RN Case Manager - Home Care Specialist

At Interim HealthCare of the Twin Cities, we are seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality care to patients in their home environment.

Responsibilities:
  • Perform assessments and procedures to evaluate patient needs
  • Develop working relationships with discharge planners and social service workers
  • Assess and review the matching of employee skills to client needs
  • Visit clients in their homes to evaluate the quality of care being provided and review clinical notes
  • Participate in ongoing interdisciplinary assessment of the patient
Requirements:
  • Graduate of an accredited school of nursing and current license as a Registered Nurse in the State of MN
  • Able to travel within a geographic area serviced by the office
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • CPR Certification
  • Two (2) years of current nursing experience
  • One (1) year case management experience preferred
