Healthy Families Support Worker Job Details
Job Summary
We are seeking a motivated and compassionate Healthy Families Support Worker to join our team at Southwestern and Affiliates. As a Healthy Families Support Worker, you will provide regular home visits to families with young children, providing support and guidance to help them achieve their goals and improve their overall well-being.
Responsibilities
* Provide home visits to families with young children, assessing their needs and developing individualized plans to support their well-being
* Work with families to identify and address risk factors that may impact their child's development
* Provide education and support to families on topics such as parenting, child development, and health and wellness
* Collaborate with other community agencies and organizations to provide comprehensive support to families
* Maintain accurate records and reports of family visits and progress
Requirements
* High school diploma or equivalent required
* 21 years of age or older
* Valid driver's license and ability to qualify for company vehicle liability insurance
* Ability to establish trusting relationships with families and work from a strength-based perspective
* Excellent communication, time management, and organizational skills
* Ability to work independently and as part of a team
