LPN - Licensed Practical Nurse - Full Time FLEX

The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to deliver compassionate and individualized care to our residents.
Responsibilities- Provide professional nursing care to residents of all ages, under the supervision of a registered nurse, advanced practice provider, or physician.
- Conduct assessments, administer medications, and provide technical support to healthcare professionals as needed.
- Document resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary.
- Counsel residents, family members, and/or resident advocates on prevention and treatment plans, under the direction of a registered nurse, advanced practice provider, or physician.
- Develop and implement self-care programs for residents, utilizing knowledge of resident education principles.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Associate degree in practical nursing is preferred.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department specific competencies and certifications.
