- Conduct thorough assessments of residents' physical and emotional needs, and develop individualized care plans to address these needs.
- Collaborate with interdisciplinary teams to ensure seamless care delivery and achieve optimal patient outcomes.
- Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making and promote a positive care environment.
- Document resident interactions, care plans, and outcomes in a timely and accurate manner.
- Graduate from an accredited practical nursing program with a current LPN license.
- Experience in long-term care or a related field is preferred.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Strong communication and interpersonal skills, with a focus on empathy and compassion.
- Ability to maintain confidentiality and handle sensitive information with discretion.
