To support the PruittHealth Hospice team in delivering nursing care to hospice patients.
Key Responsibilities
- Deliver palliative nursing care and ongoing support in accordance with the treatment plan.
- Collaborate effectively with patients, caregivers, physicians, and the interdisciplinary team.
- Implement pain management and symptom control strategies to ensure patient comfort.
- Address the physical, mental, social, and emotional needs of terminally ill patients and their families.
- Comply with all agency policies regarding payroll, billing, and documentation.
- Demonstrate knowledge of medications and their administration based on patient age and clinical condition.
- Respect and support cultural practices that do not harm others.
- Complete documentation accurately and in line with company policies.
- Refer to interdisciplinary team members and other providers as necessary.
- Educate patients and families about care provisions and services.
- Participate in Performance Improvement Programs and staff meetings.
- Provide skilled nursing care as outlined in the care plan.
- Engage in the development of interdisciplinary care plans for patients and families.
- Monitor educational needs and provide necessary training and support.
- Perform additional duties as assigned by management.
- Thorough understanding of hospice regulations at all levels.
- Excellent verbal and written communication skills.
- Strong organizational abilities.
- Ability to manage confidential information.
- Flexibility in work scheduling.
- Commitment to hospice principles and standards.
- Ability to interact with diverse age groups.
- Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
