Job Summary

We are seeking a highly motivated and compassionate Community Liaison to join our team. The ideal candidate will have a strong background in community support and patient engagement, with a passion for helping underserved populations.

Key Responsibilities

  • Community Outreach
    • Develop and maintain relationships with community stakeholders, including resource centers, medical facilities, and advocacy groups.
    • Drive referrals into Vynca care programs, primarily Enhanced Care Management.
  • Strategic Planning
    • Continuously expand knowledge and understanding of community services and resources.
    • Maintain frequent contact with community agencies serving vulnerable populations.
  • Marketing and Communications
    • Develop and execute strategic marketing plans to promote Vynca's services.
    • Identify and communicate cultural issues affecting members.
  • Customer Service
    • Support community events and maintain high customer service standards.
    • Serve as a liaison between patients and practices.

Requirements

  • 2 years of experience in healthcare sales or at a community-based organization preferred.
  • Ideal candidates will have knowledge of the local healthcare landscape and/or community resources.
  • Experience working with underserved communities and in healthcare.
  • Experience using technologies such as a computer, telephone, and various types of electronic health record (EHR) platforms.
  • Experience working in a CRM, Salesforce experience preferred.
  • Strong organizational skills, ability to work independently, and proficient with Microsoft and Google applications.
  • Strong communication, speaking, and presentation skills.
  • Strong motivational skills, including conflict/dispute resolution techniques.
  • Valid driver's license and reliable, insured, and registered automobile.
