Certified Nursing Assistant

Community Wellness Partners is seeking a compassionate and dedicated Certified Nursing Assistant to join our team. As a CNA, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met. Responsibilities:
Assist residents with activities of daily living (ADLs), including bathing, dressing, and grooming
Provide personal care, including oral hygiene, nail care, and hair care
Assist with toileting needs, including peri care and catheter care
Transfer, ambulate, and transport residents as needed
Maintain resident rooms in an orderly fashion
Serve meals, provide nourishments, and fluids as necessary
Check vital signs, including temperature, pulse, respiration, and blood pressure
Clean and maintain equipment and utility rooms as assigned
Provide post-mortem care
Obtain and record resident weights in CareTracker
Complete documentation, including AOL monitoring, special device records, I&O, toileting schedule, intake studies, and BM record
Refer visitors and family members to Charge Nurse, Unit Manager, and/or Nursing Supervisor for information regarding resident's condition
Take all breaks off floor and check with Charge Nurse before leaving the floor for any reason
Observe resident behavior and report any changes or unusual findings to Charge Nurse
Assist residents in preparing for doctor's visits, religious services, rehabilitation services, diagnostic services, and social events
Give prompt and efficient assistance to residents, answer call bells promptly
Keep incontinent residents clean and dry, change clothing and/or linen as necessary, and provide peri care as required
Change bed linen and make beds in an appropriate and timely manner
Turn and change position of resident, lift residents as needed
Attend 12 hours of in-services per year and attend nursing meetings
May accompany residents on outings and/or medical appointments
Perform other related duties as directed by Supervisor, Department Head, and/or Administrator
Qualifications:
Completion of a 100-Hour Nurse Aide Training Program
Completion of New York State Residential Health Care Facility Nurse Aide clinical skills and written exam, and proof of certification
Previous experience preferred
Sense of responsibility to job/residents, moral integrity, and understanding required
Ability to work cooperatively with others as part of a team effort
Good mental and physical health required
Sincere desire to work with the aged
Pleasant, cheerful, tactful, and cautious with residents, relatives, and visitors, as well as other staff members
Flexibility and ability to remain calm in emergencies
Treat resident information confidentially
Knowledge of resident's rights
