Key Responsibilities- Case Management: Provide education and support to clients to strengthen daily living skills, engage in outpatient care, and access community resources.
- Resource Development: Assist clients in applying for public assistance, providing supporting documentation, and connecting them with resources for vocational, financial, medical, housing, utilities, food, transportation, or basic living needs.
- Group Facilitation: Facilitate groups, classes, and workshops to promote client engagement and recovery.
- Team Collaboration: Participate in team meetings to staff cases, communicate administrative information, and collaborate with other teams on outreach, engagement, and paperwork.
- Education: Bachelor's degree in a human service field.
- Experience: Experience working with adults and/or children in an outpatient setting, leading groups, and connecting clients with community resources.
- Certifications: QMAP training and certification required for some positions.
- Driver's License: Must have a current CO Driver's License and a safe driving record.
