Job Summary:

We are seeking an experienced Inpatient Palliative Care Advanced Practice Provider to join our team at SUNY Upstate Medical University. This full-time position offers a competitive salary of $120,000 - $150,000 per year, depending on experience.

This role involves providing specialized palliative care consultation to patients requiring inpatient care. The successful candidate will have strong communication and leadership skills, with the ability to work effectively as part of a multidisciplinary healthcare team.

Responsibilities:

• Provide consultative services to identify and communicate patient goals of care
• Recommend and implement symptom management strategies
• Develop and implement acute and chronic pain management plans
• Evaluate and treat patients, provide education and guidance on palliative care principles
• Lead process improvement activities related to palliative care
• Maintain excellent relationships with families, referring providers, and healthcare team members

Requirements:

• Master's Degree as a Nurse Practitioner or Physician Assistant
• Current NYS license/registration as a NP or PA
• National Certification consistent with clinical practice
• NPI number and Medicare enrollment
• CPR-BLS Certification
• Ability to pass pre-employment and credentialing clearances
