  • Manage patient records and ensure all documentation is accurate and up-to-date, maintaining confidentiality and adhering to HIPAA guidelines.
  • Handle patient inquiries and provide information on available resources and support services to promote patient empowerment and self-management.
  • Assist in the organization and scheduling of telehealth appointments to increase access to care and reduce barriers to treatment.
Requirements and Qualifications
  • Minimum of 2 years' experience in a clinical setting, preferably in chronic care management.
  • Strong understanding of chronic diseases such as diabetes, hypertension, COPD, and heart disease, as well as their impact on patients and families.
  • Proficient in electronic medical records and able to navigate complex healthcare systems.
  • Ability to work independently and manage time effectively, prioritizing tasks and meeting deadlines.
  • Compassionate, patient-centered approach to healthcare, with a focus on empathy, active listening, and cultural sensitivity.
  • Candidates must have completed a valid U.S. MA certification program and possess a strong foundation in medical terminology, anatomy, and physiology.
  • Candidates must be able to satisfactorily complete a pre-employment background screening process (Criminal History, Licensing, etc.) and pass a pre-employment drug-screen.
Compensation and Benefits
  • Competitive compensation package, including a salary range of $18-$22 per hour.
  • 15 PTO days annually, plus 3 additional days for CME attendance.
  • $2,500 Annual CME allowance, plus a monthly phone stipend.
  • 401k with employer matching (after 90 days of employment).
  • Medical, Vision, and Dental insurance, as well as STD-LTD and Company-paid life insurance.
