Job Overview

KEY RESPONSIBILITIES

  • Deliver direct support services to children and youth experiencing serious emotional disturbances (SED) and their families.
  • Implement services in alignment with the strengths-based model.
  • Complete all mandatory training sessions for the role.
  • Undergo background checks every two years as required.
  • Collaborate with clients, families, and treatment teams to establish treatment goals, objectives, and discharge criteria. This includes facilitating the treatment planning process, documenting individualized treatment plans, and making necessary adjustments based on ongoing assessments.
  • Ensure regular communication and updates with collateral contacts through various means, including phone calls, written correspondence, and face-to-face meetings with other service providers.
  • Assist individuals in accessing essential medical, social, educational, and other services, including support with benefit applications and transportation arrangements.
  • Coordinate comprehensive service plans for children and youth, ensuring integration of family members and community resources.
  • Support families in crisis planning as appropriate.
  • Advocate for children and families, assisting them in navigating available resources and services.
  • Manage coordination of multiple services for families and youth.
  • Participate in case review meetings, case assignments, and individual planning sessions. Caseloads will be assigned and monitored by Team Leaders based on clinical needs.
  • Engage in networking activities to enhance service delivery and meet client needs.
  • Complete case management treatment plans within 30 days of family acceptance into the program, based on prior assessments.
  • Maintain timely clinical documentation, including progress notes, and stay informed of agency updates through internal communications.
  • Uphold client confidentiality and demonstrate respect for privacy.
  • Act as a representative of Family Service and Guidance Center within the community, which may involve educational presentations.
  • Support and promote the mission and vision of Family Service and Guidance Center, emphasizing a strengths-based approach.
  • Perform additional duties as assigned by supervisors or team leaders.

QUALIFICATIONS

Education and Licensure:

  • Bachelor's degree in a human services field or equivalent experience in a related area.
  • A valid driver's license, current auto insurance, a good driving record, and access to a reliable vehicle are mandatory.

Experience:

  • Preferred experience working with children and families.
  • A strong passion for supporting children and adolescents.

Frequent interruptions may occur, and staff are expected to handle crisis situations, including potential exposure to aggressive behaviors from children and youth, as well as interactions with families from diverse socioeconomic backgrounds.
