Job Summary

We are seeking a highly skilled and compassionate School Based Case Manager to join our team at Family Service and Guidance Center Topeka. As a key member of our team, you will work closely with children, families, and school districts to provide essential support services and promote positive outcomes.

Key Responsibilities
  • Provide direct services to children and families, including individual and group counseling, case management, and advocacy.
  • Collaborate with school districts to develop and implement individualized treatment plans that address the unique needs of each child.
  • Work closely with school personnel, clinicians, and other stakeholders to ensure seamless communication and coordination of services.
  • Engage with children and families in the classroom, home, or community to build skills and promote positive relationships.
  • Make referrals to community resources and assist with the application process for benefits and services.
  • Coordinate services for children and families, ensuring an integrated and comprehensive plan of supports and services.
  • Act as an advocate for children and families, assisting with crisis planning and ensuring access to needed resources and services.
Requirements
  • Bachelor's degree in a human service field or equivalent experience.
  • Valid driver's license, current auto insurance, and a good driving record.
  • Ability to pass background investigations, including KBI and DCF Central Registry checks.
  • Experience working with children and families, with a passion for supporting their well-being and success.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
Work Environment

This is a full-time position, requiring a flexible schedule and the ability to work in a fast-paced, dynamic environment. The School Based Case Manager will work in an office and community-based setting, with frequent interruptions and exposure to aggressive and/or acting out children and youth.
