Key Responsibilities- Case Management: Develop and implement comprehensive care plans for clients, coordinating services to meet their unique needs.
- Client Selection and Assessment: Conduct initial screenings and enrollment visits to assess client eligibility and suitability for our programs.
- Record Keeping: Maintain accurate and up-to-date records of client interactions, assessments, and care plans.
- Education: Master's degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology, or a related field.
- Experience: At least one year of experience working with the elderly, preferably in a care management or social services role.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
