The Licensed Practical Nurse is a vital member of our healthcare team, responsible for providing high-quality patient care and collaborating with other professionals to ensure optimal outcomes.
Key Responsibilities- Participate in the nursing process by providing input to the client's plan of care.
- Implement the care plan as written and evaluate its effectiveness, making recommendations for revisions as necessary.
- Record all pertinent observations and treatments as per agency policies.
- Prepare clinical progress notes and provide appropriate documentation of service in keeping with agency policies and procedures.
- Provide for the emotional and physical comfort and safety of the client.
- Perform nursing procedures for which he/she has been trained.
- Notify appropriate personnel about unusual symptoms and/or changes in the client's condition.
- Participate in case conferences, team meetings, and performance improvement activities as assigned.
- Provide proof of current licensure by the state.
- Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA.
- Must have one year of current experience in an accredited hospital or accredited nursing home within two years previous to employment.
