Job Title: Home Care Professional

Overview: Community Residences is seeking a skilled and compassionate Home Care Professional to join our team. As a key member of our community, you will provide essential support and care to individuals with developmental disabilities in a residential setting.

About the Role: The successful candidate will be responsible for creating a nurturing environment, developing strategies to promote independence, and implementing individualized plans of care. You will work closely with our interdisciplinary team to identify client needs, record progress, and document any barriers that may impact their growth.

Key Responsibilities:
  • Create a positive learning environment and develop strategies to promote independence.
  • Implement individualized plans of care, record data on teaching strategies, and report progress.
  • Monitor medical, physical, and behavioral changes, and report any concerns to supervisors.
  • Administer medication, as prescribed, and maintain accurate documentation.
  • Assist with daily living tasks, such as meal preparation, housekeeping, and transportation.
  • Develop a positive neighborhood/community relationship and participate in training opportunities.

Requirements: To be successful in this role, you will require a high school diploma or equivalent, at least two years of experience in human services, and a valid driver's license. A Bachelor's degree in a related field is preferred.

Working Hours: This position requires flexible working hours, including evenings, weekends, and some overnight shifts. A competitive salary is offered, estimated at $38,500 per year, based on location and experience.
