Job Summary

We are seeking a highly skilled Licensed Vocational Nurse to serve as our Wellness Director at Glen Park Senior Living. As a key member of our care team, you will be responsible for planning, organizing, and directing the provision of resident care activities within our facility.

Key Responsibilities
  • Develop and implement resident care plans in accordance with established practices and policies.
  • Supervise and evaluate the delivery of timely and appropriate high-quality assistance, care, and medication management to all residents.
  • Collaborate with outside medical professionals, regulatory agencies, and suppliers to ensure the highest level of care.
  • Perform assessments on admission and periodically during residency.
  • Monitor residents for prohibited health conditions and report any changes to administration.
  • Complete weekly progress notes and monthly head-to-toe assessments for all residents.
Requirements
  • Graduate of an accredited School of Nursing.
  • Legally-licensed by the State Board of Nursing to practice as a Licensed Vocational Nurse.
  • Experience as an LVN with a minimum of 1 year of acute care experience.
  • Current CPR and First Aid Card.
  • Health/physical clearance within 6 months; TB test clearance.
  • Criminal Background clearance.
  • Good written and verbal communication skills.
Working Conditions

The ideal candidate will be physically able to lift and pull up to 20 lbs., bend, stretch, and climb stairs. Support residents in turning, standing, sitting, transferring, and walking activities.
