We are looking for a motivated and proactive **Support Services Administrator** (Case Manager/Social Worker).

The **Wood County Board of Developmental Disabilities (WCBDD)** is dedicated to coordinating support for individuals with developmental disabilities in Wood County. Our vision is to empower and uplift those we serve. The **Support Services Administrator** will oversee a caseload of clients, develop individual service plans, and monitor the delivery of services to ensure safety and well-being.

Position Details:
  • Classification: Civil Service Appointment
  • Work Hours: Full-Time, 80 hours bi-weekly
  • FLSA Status: Non-Exempt
  • Healthcare Benefits: Available
  • Standard Working Hours: Monday to Friday, 8:00 AM - 4:30 PM
Key Responsibilities:
  • Organize and lead Individual Service Plan (ISP) and Individual Family Service Plan (IFSP) meetings with a focus on person-centered approaches.
  • Draft and modify the ISP/IFSP as necessary, including service authorizations based on assessed needs and through the Interdisciplinary Team process.
  • Responsible for the implementation, documentation, and monitoring of ISP data, including narratives and assessments, ensuring compliance with state and federal regulations.
  • Evaluate, coordinate, and ensure the delivery of services tailored to individual needs as outlined in the ISP.
  • Guide all services and behavioral programs effectively.
  • Ensure thorough and precise documentation of all programs and service delivery reviews.
  • Facilitate individual/guardian choice in provider selection, assisting as necessary in accordance with HCBS guidelines and Board policies.
  • Collaborate with local educational institutions and Early Intervention providers.
  • Act as a liaison among Board components, families, external agencies, the community, and contracted service providers.
  • Provide information and assistance in accessing services for eligible individuals in educational, vocational, housing, and community-based programs.
  • Advocate for eligible individuals through direct engagement and document all service and support activities via Targeted Case Management.
  • Offer information and referral services to interested individuals, coordinating services through direct contact and sharing information within the agency and with other providers.
  • Review unusual incidents and monitor for trends and patterns.
  • Assist individuals and families in completing all necessary documentation for initial Waiver enrollment, ensuring accuracy before submission.
  • Support individuals and families in coordinating services based on comprehensive assessments.
  • Coordinate initial and annual evaluations/assessments as required.
  • Maintain ongoing communication with individuals, families, and providers regarding the status of services and ISP updates.
  • Stay informed about HCBS Waivers.
  • Provide technical support to individuals and families regarding billing, documentation, and service modifications.
  • Assist in developing success strategies and monitor the effectiveness of programs, participating in Transdisciplinary Meetings.
  • Help complete forms for review by the Human Rights Committee regarding restrictive measures.
  • Share on-call responsibilities with other Support Services Administrators for crisis intervention and stabilization.
  • Possess knowledge of Major Unusual Incidents (MUI) and related reporting requirements.

  • Minimum of 2 years of experience working directly with individuals with developmental disabilities preferred.

Minimum Qualifications:
  • Certification from the Ohio Department of Developmental Disabilities, if applicable.

  • Pre-employment and random drug and alcohol testing required.
  • Satisfactory criminal background checks required.
  • Must be able to lift and assist individuals receiving services safely.
  • Ability to interact appropriately with individuals displaying challenging behaviors.
  • Strong organizational skills to manage multiple projects and deadlines.
  • Ability to communicate effectively and maintain confidentiality.
Physical Demands and Work Environment:
  • Work primarily in an indoor setting, with occasional community engagement.
  • Physical ability to stand or sit for extended periods, lift moderate weights, and operate office equipment.
  • Required to safely move individuals using proper techniques.
