About the Role

Swope Health Services is seeking a skilled Behavioral Health Consultant to join our team of dedicated caregivers. As a key member of our Primary Care Team, you will provide critical support to our clients, addressing their psychosocial needs and promoting overall well-being.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments for children, youth, and adults in Primary Care, identifying areas of need and developing targeted interventions.
  • Collaborate with our Primary Care Team to develop and implement integrated treatment plans, ensuring seamless care coordination and optimal outcomes.
  • Engage individuals and families as partners in services, fostering positive relationships and empowering them to achieve their goals.
  • Monitor and evaluate client progress, modifying treatment plans as needed to ensure the best possible results.
  • Participate in interdisciplinary treatment team meetings, contributing to a comprehensive care model that addresses the unique needs of each client.
Requirements
  • Licensed Clinical Social Worker (LCSW), Licensed Clinical Psychologist (LCP), or a PhD in Clinical Psychology is preferred.
  • A Licensed Professional Counselor (LPC), Licensed Master's Social Worker (LMSW), Provisionally Licensed Professional Counselor (PLPC), or other Master's Level Clinicians in the Behavioral Health Field, preferably seeking licensure.
  • One to Three years of professional experience working with seriously and persistently mentally ill adults, with a focus on clients who have experienced trauma, homelessness, and/or substance abuse.
  • Proficient knowledge of the DSM-5/ICD-10, with the ability to assess mental health disorders and make person-centered treatment and service recommendations.
  • Proficient knowledge of health and human services and community resources.
