Care Management Services Director

At CHSGa, we're committed to delivering the highest level of care in our communities. As a Director of Assessments for Care Management Services, you'll play a vital role in ensuring our patients receive the best possible care.
Key Responsibilities:- Lead the assessment process for Care Management Services, ensuring timely and effective assessments of potential members and members.
- Collaborate with leaders across various departments to ensure seamless admissions and transitions.
- Develop and implement best practices for assessment and admissions, supervising LPN staff and ensuring adherence to regulatory guidelines.
- Oversee the admission process for assigned care management operational regions, ensuring promptness and quality metrics are met.
- Work closely with the Health Care Navigation team to ensure effective transitions of patients in the CHSGa system.
- Participate in team meetings and contribute to the development of action plans to address key performance indicators.
- Ensure effective communication between Assessment Nurses, Assessment Coordinators, and Health Care Navigators regarding patient admissions and transitions.
- Monitor and analyze data to ensure appropriate staffing and process improvements.
- RN license with a minimum of 5 years of experience in the health and human services field, with 5 years of supervisory experience.
- Bachelor's degree in Social Work or related field with a minimum of 5 years of experience in the health and human services field, with at least 5 years in management or supervisory position.
- Experience in social work, home and community based services, healthcare, or geriatrics preferred.
- Valid Driver's License and reliable transportation.
