Care Management Assistant

Summa Health is seeking a highly organized and detail-oriented Care Management Assistant to provide administrative support to our licensed care management team.

Key Responsibilities:
  • Provide clerical assistance with care coordination and transition planning efforts
  • Support efficient movement of patients to post-acute facilities, including Skilled Nursing Facilities, Long-term Acute Care Hospitals, and Intermediate Care Facilities
  • Assist with arranging transportation to facilities or arranging follow-ups, as directed by licensed staff
  • Work collaboratively with internal multidisciplinary teams, providers, allied health staff, and external community providers/agencies
Requirements:
  • High school diploma or equivalent
  • Minimum one year experience in an administrative or business setting, with experience supporting colleagues and working with data
  • Basic experience using a computer and previous experience in healthcare preferred
  • Knowledge of medical terminology and electronic health record (EHR) systems preferred
