Job Summary

Family Service League is seeking a full-time Case Manager for the Adult Care Management State Funded program. The Case Manager will provide intensive support to individuals with serious mental illnesses and other diagnoses, meeting visit requirements as stipulated by OMH. The ideal candidate will have a strong assessment, engagement, and organizational skills, with excellent verbal and written communication skills.

Responsibilities

  • Maintain a positive working rapport with Suffolk County Adult SPOA
  • Employ a culturally sensitive and proactive outreach approach to potential members
  • Manage a caseload of 25-35 clients and meet their intensive needs
  • Meet minimum visit requirements based on client's determined acuity level and needs
  • Speak with mental health, medical, and substance abuse providers, as well as hospitals, jails, and housing/shelter providers
  • Form and maintain positive and professional relationships with internal and external providers to coordinate care and obtain needed information
  • Complete all required documentation in a timely and accurate manner
  • Report all incidents to program management staff
  • Follow all agency and program mandates as set by FSL, Department of Health, and Suffolk County Division of Mental Hygiene

Qualifications

  • Bachelor's Degree in a health-related field required
  • Minimum of 2 years of experience providing direct services to people with serious mental illness required
  • Proficiency in Microsoft Office required; experience with an EHR is preferred
  • Strong assessment, engagement, organizational, and time management skills
  • Excellent verbal and written communication skills required
  • Valid and clean NYS Driver's License required
