We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at Post Health at Home in the North Houston Area. As a PRN LVN/LPN, you will have the opportunity to work in a dynamic and fast-paced environment, providing high-quality patient care and making a meaningful difference in our community.

Responsibilities
  • Provide skilled nursing care to patients in their homes, promoting independence and improving quality of life.
  • Conduct thorough assessments, develop and implement individualized care plans, and evaluate patient progress.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Administer medications, perform wound care, and provide education to patients and families.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
  • Active LPN/LVN license from a board-certified nursing school.
  • Proof of license to practice in the state serviced.
  • One year of clinical experience.
  • Proof of eligibility to pass medications.
  • CPR or BLS Certification.
  • Valid Driver's License.
