Overview

Lutheran SeniorLife is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes. As a Home Health RN, you will be responsible for delivering comprehensive nursing care, managing patient conditions, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Supervise and educate home health aides, ensuring they provide high-quality care and support to patients.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that enhance patient care and outcomes.
Requirements
  • Current licensure as a Registered Nurse in the Commonwealth of Pennsylvania.
  • One year of experience in hospital medical/surgical nursing, public health nursing, or related experience preferred.
  • Prior home health care, hospice, psychiatric, and/or mental health nursing experience preferred.
  • Dependable transportation with a valid driver's license.
  • Working knowledge of computers, Microsoft Word, and Excel.
