Job Summary

We are seeking a compassionate and experienced Patient Care Liaison to join our team at Heart to Heart Hospice. As a Patient Care Liaison, you will play a vital role in promoting a customer-focused hospice presence in the healthcare community.

Key Responsibilities
  • Coordinate care amongst healthcare professionals to ensure seamless patient care.
  • Ensure patients' Durable Medical Equipment (DME) is ordered, delivered, and in good working condition, with proper training for patients and family members.
  • Communicate regularly with patients and family members to ensure all needs are met and satisfaction is high.
  • Identify and resolve problems or dissatisfaction with patients and/or family members, communicating effectively with the clinical team and management as needed.
  • Coordinate and provide in-services or educational events for family members and/or facilities.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Vocational Nursing.
  • Current State license as a Licensed Vocational Nurse or Licensed Practical Nurse preferred.
  • Valid driver's license and car insurance.
  • Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting, preferably in hospice or home health.
  • Customer service experience strongly preferred.
