Key Responsibilities

  • Assist with Daily Living Activities: Assist patients with bathing, dressing, grooming, and mobility, promoting their independence and dignity.
  • Monitor and Report Health Status: Monitor patients' health status and report any changes to the supervising nurse, ensuring timely interventions and optimal care.
  • Provide Emotional Support: Offer emotional support and companionship to patients and their families, fostering a sense of comfort and well-being.
  • Document Care Provided: Maintain accurate and timely documentation of care provided, ensuring seamless communication with healthcare professionals.
  • Ensure a Safe Environment: Ensure a safe and clean environment for patients, adhering to infection control protocols and promoting a healthy living space.
Requirements
  • Valid CNA License: Possess a valid Certified Nursing Assistant (CNA) license in the state of Pennsylvania.
  • CPR Certification: Hold current CPR certification.
  • Experience in Home Health or Hospice Care: Previous experience in home health or hospice care is preferred, but not required.
  • Strong Communication Skills: Demonstrate strong communication and interpersonal skills, ensuring effective collaboration with patients, families, and healthcare professionals.
  • Ability to Work Independently: Possess the ability to work independently and as part of a team, adapting to changing situations and priorities.
  • Compassionate Nature: Exhibit a compassionate, patient, and empathetic nature, providing exceptional care and support to patients and their families.
  • Reliable Transportation: Possess reliable transportation and a willingness to travel to patient homes within the service area.
