Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will work in post-acute facilities in the East Haven, Connecticut area, providing high-quality patient care and support. Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and conducting intakes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • At least 6 months of experience (LPN or similar role)
